Groups Plans for Companies with 2 or More
Employees
In order to provide an accurate quote for group health, dental,
or other benefits, we require your employee census. It should
contain the following for each employee:
- Employee Name
- Gender
- Birth date
- Social Security Number
- Coverage (Individual, Employee +1, Family)
- Salary (If requesting quotes on Life or
Disability Products)
If you currently have group benefits
for your company, copies of the following will give us the best
chance to provide a quote and know if we can offer a cost-effective
solution for your company:
- Maryland Quarterly Unemployment
Tax Statement. (You will want to note additions and
deletions to your staff since the last statement, and indicate any
employees who have other coverage.)
- Most recent bill for your group
health insurance.
- Your current group's Benefit
Plan Summary.
