Accidental Loss or Dismemberment Benefits Plan provides:

90 Day Loss Limit Restriction - The loss must occur within 90 days of the date of the accident. In the event of more than one loss from any one accident, payment will only be made for the loss with the highest amount payable.

Seat Belt Benefit - Included at no extra cost - Pays an additional 50% of the benefit otherwise payable (up to a maximum of $50,000) if the loss is caused by an accident occurring while the insured person is wearing a seat belt. (Benefit not available in Minnesota)

Common Carrier Double Benefit Allowance -Up to $100,000 additional benefit will be paid for loss of life caused by an accident that happens anywhere in the world while you are a fare-paying passenger on a public conveyance operated by a licensed common carrier, including bus, train, boat and airplane. That’s double your basic benefit amount.

Education Benefit - Included at no extra cost - If dependent child(ren) coverage is elected and the member dies while dependent child(ren) is/ are enrolled in an institute of higher learning (i.e. college or university), an additional 2% of the death benefit (to a maximum of $2,500) will be paid annually to each eligible dependent child for a maximum of 4 years, provided the child continues his/ her education.

Your Choice of Beneficiary - Choose anyone you wish to be your beneficiary.  The Member is automatically the beneficiary for child(ren)’s coverage.  You may change your beneficiary at any time by writing Benefit Choices who markets this plan.  Claim payments are made promptly upon proof of death or loss.  Your beneficiary may choose to receive the benefit at one time or in monthly installments.

Renewal and Conversion Rights - You can not be singled out for cancellation.  You may continue your Accidental Death & Dismemberment Insurance coverage to age 70 assuming you pay your premiums and the Group Policy remains in force.
